    Hypertension is an abnormal increase in blood pressure in arterial blood vessels continuously for more than one period. This occurs when the arterioles are constricting. Arteriole constriction makes blood difficult to flow and increases pressure against arterial walls. The prevalence of hypertension in West Sumatra province reaches 21.4%. Data from the Provincial Health Office of West Sumatra in 2017 hypertension is the 10 most common diseases suffered by the community with 69,500 people suffering from hypertension. Pharmacological therapies available for hypertension have not shown improvement in reducing blood pressure in hypertensive patients. Very necessary companion therapy that is non-pharmacological treatment is expected to be able to complete pharmacological treatment in reducing blood pressure by using noni juice juice (Morinda Citrifolia).This study used the design of Quasy Experiment with the approach of group pretest posttest design. This research was carried out in the Belimbing Health Center Working Area The number of samples in this study were 10 respondents with purposive sampling technique. Data analysis in this study using univariate and bivariate using dependent t-test. The results obtained, the average systolic blood pressure of 12 mmHg with a value (p Value 0,000) and diastolic 5 mmHg (p Value 0.015) means that there is an influence on the average blood pressure before and after being given Noni juice.Based on the results of the research that has been obtained, it is expected that health workers through the head of the Puskesmas can apply or use Noni fruit juice as an alternative treatment for hypertension

    Graphene oxide-polysulfone hollow fibers membranes with synergic ultrafiltration and adsorption for enhanced drinking water treatment

    Polysulfone-graphene oxide hollow fiber membranes (PSU-GO HFs) with simultaneous adsorption and ultrafiltration capabilities are herein described and proposed for enhanced and simplified Point-of-Use (POU) drinking water purification. The PSU-GO HFs were prepared by phase inversion extrusion by a customized semi-industrial plant and their morphology, surface properties, and porosity were investigated by combined Scanning Electron Microscopy (SEM), contact angle and Raman confocal microscopy, in relation to different GO:PSU ratios (1–5% w/w GO vs PSU) and to the final adsorption-ultrafiltration properties. Filtration modules of PSU-GO HFs of filtering surface (FS) in the range 0,015–0,28 m2 showed same ultrafiltration capability of PSU-HF standard filters. Synergic adsorption properties were demonstrated by studying the adsorption maximum capacity of ciprofloxacin antibiotic (CIPRO) vs GO ratio in dead end in-out configuration, the standard configuration used for PSU HFs commercial modules. Loading of 3,5% GO vs PSU was selected as case study, representing the best compromise between performance and GO nanofiller amount. Heavy metals (Pb, Cu and Cr(III)) and polyfluoroalkyl substances (PFAS) removal capabilities from tap water were competitive and in some cases outperformed Granular Activated Carbon (GAC), the standard industrial sorbent. Ciprofloxacin removal from tap water was also under real operational conditions. Moreover, release of GO from working PSU-GO modules was excluded by Surface Enhanced Raman Spectroscopy (SERS) analysis of treated water having the state-of-the-art limit of quantification of 0.1 μg/L for GO nanosheets

    Protein components in food can trigger immune-mediated response in susceptible individuals. International law requires risk assessment to be undertaken by competent individuals to minimize food safety risk to consumers. Historically, allergen control legislation has been food focused and on the requirement for on pack labeling, and the need for formal food recalls in the event of misleading or inappropriate labeling. In order to develop a mechanism for decision makers when assessing allergenic risk from plant derived materials, the aim of this research was to consider a more holistic risk assessment method whereby rather than just using the food-based approach, an additive element in terms of considering the families of proteins is included. This approach reflects the need for food professionals to fully understand the role of proteins in triggering an allergic response to plant material and the health risk to individuals who show cross-reactivity to such proteins

    Glycation marker glucosepane increases with the progression of osteoarthritis and correlates with morphological and functional changes of cartilage in vivo

    Background: Changes of serum concentrations of glycated, oxidized, and nitrated amino acids and hydroxyproline and anticyclic citrullinated peptide antibody status combined by machine learning techniques in algorithms have recently been found to provide improved diagnosis and typing of early-stage arthritis of the knee, including osteoarthritis (OA), in patients. The association of glycated, oxidized, and nitrated amino acids released from the joint with development and progression of knee OA is unknown. We studied this in an OA animal model as well as interleukin-1β-activated human chondrocytes in vitro and translated key findings to patients with OA. Methods: Sixty male 3-week-old Dunkin-Hartley guinea pigs were studied. Separate groups of 12 animals were killed at age 4, 12, 20, 28 and 36 weeks, and histological severity of knee OA was evaluated, and cartilage rheological properties were assessed. Human chondrocytes cultured in multilayers were treated for 10 days with interleukin-1β. Human patients with early and advanced OA and healthy controls were recruited, blood samples were collected, and serum or plasma was prepared. Serum, plasma, and culture medium were analyzed for glycated, oxidized, and nitrated amino acids. Results: Severity of OA increased progressively in guinea pigs with age. Glycated, oxidized, and nitrated amino acids were increased markedly at week 36, with glucosepane and dityrosine increasing progressively from weeks 20 and 28, respectively. Glucosepane correlated positively with OA histological severity (r = 0.58, p < 0.0001) and instantaneous modulus (r = 0.52–0.56; p < 0.0001), oxidation free adducts correlated positively with OA severity (p < 0.0009–0.0062), and hydroxyproline correlated positively with cartilage thickness (p < 0.0003–0.003). Interleukin-1β increased the release of glycated and nitrated amino acids from chondrocytes in vitro. In clinical translation, plasma glucosepane was increased 38% in early-stage OA (p < 0.05) and sixfold in patients with advanced OA (p < 0.001) compared with healthy controls. Conclusions: These studies further advance the prospective role of glycated, oxidized, and nitrated amino acids as serum biomarkers in diagnostic algorithms for early-stage detection of OA and other arthritic disease. Plasma glucosepane, reported here for the first time to our knowledge, may improve early-stage diagnosis and progression of clinical OA

    <p>Abstract</p> <p>Background</p> <p>Several studies document disparities in access to care and quality of care for depression for African Americans. Research suggests that patient attitudes and clinician communication behaviors may contribute to these disparities. Evidence links patient-centered care to improvements in mental health outcomes; therefore, quality improvement interventions that enhance this dimension of care are promising strategies to improve treatment and outcomes of depression among African Americans. This paper describes the design of the BRIDGE (Blacks Receiving Interventions for Depression and Gaining Empowerment) Study. The goal of the study is to compare the effectiveness of two interventions for African-American patients with depression--a standard quality improvement program and a patient-centered quality improvement program. The main hypothesis is that patients in the patient-centered group will have a greater reduction in their depression symptoms, higher rates of depression remission, and greater improvements in mental health functioning at six, twelve, and eighteen months than patients in the standard group. The study also examines patient ratings of care and receipt of guideline-concordant treatment for depression.</p> <p>Methods/Design</p> <p>A total of 36 primary care clinicians and 132 of their African-American patients with major depressive disorder were recruited into a cluster randomized trial. The study uses intent-to-treat analyses to compare the effectiveness of standard quality improvement interventions (academic detailing about depression guidelines for clinicians and disease-oriented care management for their patients) and patient-centered quality improvement interventions (communication skills training to enhance participatory decision-making for clinicians and care management focused on explanatory models, socio-cultural barriers, and treatment preferences for their patients) for improving outcomes over 12 months of follow-up.</p> <p>Discussion</p> <p>The BRIDGE Study includes clinicians and African-American patients in under-resourced community-based practices who have not been well-represented in clinical trials to improve depression care. The patient-centered and culturally targeted approach to depression care is a relatively new one that has not been tested in most previous studies. The study will provide evidence about whether patient-centered accommodations improve quality of care and outcomes to a greater extent than standard quality improvement strategies for African Americans with depression.</p> <p>Trial Registration</p> <p>ClinicalTrials.gov NCT00243425</p
